    Anaesthesia/Critical Care here. As though the internist actually understood any pathophysiology that takes less than two months to take place... By the time he's done with his literature review on whether to up the norepi, the patient's already been saved from him by an anaesthesiologist three times over. Go back to your ward, COPD bro, considering that on a daily basis, I AM the patient's physiology, I think I can handle this. (Srsly though, 95% of internists don't know the first thing about haemodynamics or how to assess volume status. A frightening amount can't even do a basic bedside echo. Literally three hours ago I saved someone from a hoarde of them.)

    I’ve been in high dependency unit before on CPAP - continuous positive airway pressure. Is CPAP same as PEEP?

      CPAP is a ventilation mode, PEEP, positive end-expiratory pressure, is a pressure parameter that is found in many ventilation modes, including CPAP. Strictly speaking, CPAP means that the ventilator will provide you with a positive pressure at every stage of your breathing cycle. So to speak, you only set a PEEP and no other pressure. There is also CPAP/PS (also CPAP/ASB or others, depending on the manufacturer of the ventilator), where in addition to the PEEP you get a bit of a boost, pressure support, from the ventilator each time you take a breath. Even in fully controlled modes, such as BIPAP, PCV, VCV etc. one can also set a PEEP. In those cases PEEP helps to pre-dilate the lungs to make the breathing/ventilating itself easier and it keeps areas of the lung that are prone to being blocked shut open. In difficult to ventilate patients, the PEEP is something we use quite intensely.

    This raises the existential question: How many medical students have been murdered in teaching hospitals and their very existence covered up? As I think about it, I am pretty sure I had a couple college roommates who went to medical school; yet when I look at my college yearbook it looks like their pictures and names have been scribbled over with a black sharpie .... I am scared. Check on me in a couple days. If my profile disappears, either the surgery charge nurses or the respiratory therapists got to me. Oh no! What if they are working together?

    As a trainee anaesthetist with an interest in critical care in the UK, I've found that the opposite is true RE medical Vs Anaesthetic intensivists. I think the medical ones do it to get away from the complex med, so they LOVE the surgical ones. Meanwhile every anaesthetist who missed doing diagnostics leaps at complex medicine.

      Anaesthesia/critical care: I love interdisciplinary ICUs. In Germany, especially in smaller hospitals, anaesthesia tends to do critical care for all patients. In my particular hospital there is a medical ICU, but they are small and often they either overflow to us or at some point they realize they don't have the slightest clue what they are doing and transfer a patient to us. On the ICU we are pretty much GPs on coke but with much less talking involved. We place more chest drains than surgery, we do more echos than cardiology, we do more bronchoscopies than pulmonology and we can damn well poke into anything anywhere in the body.

    I usually love the characterization of Anesthesia, but... Hey, as an anesthesiologist, I take care of those complex medical ICU patients in the OR and in Radiology and give them back the ICU in the same or better condition. I don't stay out of the medical ICU because I'm scared of those patients - I do it because they don't usually need the extra expertise of an anesthesiologist!

      Thank you, dear colleague. Nobody understands the pathophysiology of a critical patient like an anaesthesiologist, because on a daily basis we ARE the patient's physiology. No other specialty has such close contact and such immediate influence on the most basic vital functions. Literally three hours ago I saved a guy from a hoarde of internists who considered "having seen something move rhythmically in the chest" as an echo and wanted to flood that poor, perfectly euvolaemic man with amidst a STEMI.

    I love how he said that he leaves the critical care pulmonology patients in the ER for 87hrs and then complains that they did it all wrong! That's so true! Poor ER nurses who once were primadonnas are now having to be treated like all the other nurses! They fled from floor nursing and ICU to ER to avoid this kind of dump on you treatment!     I touched the vent numerous times and I lived to tell the secret… I just stare down RT while holding my neurology Trömner hammer in strike pose 😂 Mostly I dial down the vent rate to 6 for 30 secs to see if patients’ medulla trigger the vent.
